Please note that to speed the process of a registration and to avoid rejection and/or duplication
You may be asked to provide the information below for each person:
- Your first UK address when registered with a GP
- If previously resident in UK, date of leaving & Date of entry in the UK
- Photo ID (Passport, Driving License etc.)
- Proof of address (Bank Statement, utility bills and etc.)
- Your UK mobile number
- We will not be able to register you if you do not provide an UK number

Please note it is important that you complete thoroughly all the questions and you tick all the boxes in the registration papers to avoid any delay in your registration. Please note that in order to register a children, at least one of the parent must be registered with us too.
If you are registering as a temporary resident your health records remain at your usual practice and are not transferred either as paper or via GP2GP. Your temporary practice will contact your registered GP if they require any information.
GP2GP enables patients’ electronic health records to be transferred directly and securely between GP practices. It improves patient care as GPs will usually have full and detailed medical records available to them for a new patient’s first consultation.
